Is it under load or braking it's most noticeable, or does it follow camber or poor road surfaces?

Yes 100% more noticeable when accelerating or braking and on poor road surfaces:

Mine used to do that, turns out the top mount bearing had failed.

How can I check this top swivel pin bearing? When hacked up and on axle stands the wheel/steering feels solid.
