High speed crank sync lost fault + Injector 3 open circuit

I HAD a problem with my throttle completely cutting out after a high acceleration. Narrowed it down to dodgy wiring from the Crank Position Sensor.

Put a new sensor in + Loom from Empire tuning.

Managed a 5 hour motorway drive (x2) no problems. About a week later, I drove for 10 minutes on the Motorway, suddenly starts to run really rough, MIL light + black smoke out the back. (was going 55mph)
Turned ignition on and off, runs and idles fine. No sign of a misfire (also no misfire code)

Did a code read and got these:
- High speed crank synchronisation Lost fault
- Injector 3 Open Circuit Detected.

Starts perfectly & runs perfectly at low speed/low temp.

Not had problems with Injectors before. Could this be a dodgy Crank Position Sensor? Or is there something worse going on?

I'm running a 2006 Def 90 TD5 @ 178,000

Check wires between ECU and injectors. Often pins in connector of the injector loom went bad, loosing contact. I have repaired several of these just by tightening these pins. Bad contact leads to open circuit.
This has no direct connection to crank position sensor. "Drobek" = The Small One - Discovery 2, "Blufínek" = The Blue Thing - Defender 130, and for me at least Ford Mondeo
