And most transits have a hard life, Take that video you've posted a link to. It might only have 52'000 on the clock, but it's an Ex-Royal mail van, even if it's not the second time around the clock, it's been used for stop start work and thrashed to the red line day in day out all day every day. Transits also have a dual mass flywheel, they fail and sound like that - the Puma Defender doesn't have a DMF.

First thing to try would be to remove the fan belt and run and see if it goes. Lots of whirring things in there.....

Well, for what it's worth - crappy youtube audio means high potential for misdiagnosis - it does sound like mine did before I 'fixed' it with the fuel related diagnostic tool stuff (VCV and pilot correction).

Next problem is that I wasn't methodical about it, so I don't actually know what the root issue is/was. Perhaps the VCV in the first case, and perhaps the loose injector clamp (that I replaced) in the second case both meant the ECU parameters learned in the last session were then out ...

Maybe ask your indie a few questions to sniff out if these processes are a part of their workflow? When getting my injectors cleaned up and reseated last year the indie I used frowned a little when I said it, but then said he would do it 'yip yip, all done!', but the noise remained, when I tried the process myself (using the BAS remote tool) I got errors, from which I finally found that the MAF temp sensor ('IAT1') was disconnected due to a misaligned plug - all indicating that it would have been impossible for the indie to do it (!).

When I finally got it done myself, it was smoooooth again. Solved the bowel problem, working on the consonants...

Well the verdict is in - not good news.
No fault codes
VCV. replaced learning done on standard tune.
Injectors OK.
Cam carrier, cams, lifters, timing chain OK.
Serpentine belt removed - noise still there.
That leaves - little end, big end, piston related.
It's now done 110k miles and been serviced per schedule using genuine parts only.
I'm heading for a recon engine, or having mine rebuilt.
Any recommendations for suppliers?
