In practical terms, nothing - they're interchangeable. Sites like Paddocks will advertise the different types as correct for the years to which they were originally fitted though.

The holes for the doors to fit into are all the same, so you can fit later ones on to earlier cars. The problem is a lot of the fittings on the doors are different. So if you just bought a bare door panel, you'd find your windows and lock mechanisms dont fit the new door.
The late type safari door for example has a totally different window glass and seal to the earlier ones. So when the likes of Paddocks sell a door panel, they're assuming you want it to replace a rotten one on your car and you'll be reusing all your existing fixtures and fittings. Then of course some people want their car to look totally original rather than fit later upgraded parts. |

You're right about most of the parts but the door locks are interchangeable. I've just fitted an earlier lock to my late rear door to retain a common key barrel. The lock just bolted straight on. I was also pleasantly surprised to find that the stay on the new door matched up with the existing bracket on the body. The bracket for the wiring loom is the same, too.
Other parts such as the wiper motor, internal grab handle, door card and obviously the spare wheel carrier are all completely different though. I too have just put a late 2002 onwards rear door on my '95 300Tdi.
With a little ingenuity the wiper motor changed over as well (easier on later 300Tdi and TD5 as the motor is the same as the new one). Door card, glass and seal and grab handle are different, lock swaps over fine as LRA says. The exception to the any door will fit is I believe the series ones which only had two hinges instead of three.
